Responsibilities:
- Engage community social networks to collaborate with YLDHC activities
- Coordinate, organize, and participate in scheduled outreach activities to facilitate member enrolment for DHC programs
- Conduct patient empowerment programs for individuals or in groups
- Assist in the promotion and publicity activities of the DHC
- May need to take on flexible hours and occasional Evening, Saturday or Sunday duties
Requirements:
- Registration with the Social Workers Registration Board
- 2 or more aggregate years of practice as Assistant Social Work Officer
- Knowledgeable in community resources and potential collaborators
- Experience in handling cases with chronic diseases and conducting patient empowerment programs will be an advantage
