Registered Nurse

Responsibilities:

• Perform health assessments for registered clients of the Yuen Long District Health Centre (YLDHC)
• Provide health education to healthy individuals to engage in healthy lifestyles to avoid risk factors relating to chronic diseases, and coaching in lifestyle modification
• Encourage clients to participate in appropriate self-management support programs
• Coordination of care among multi-disciplines in YLDHC and the YLDHC Network Service Providers
• Act as Case Manager and run Nurse Clinic
• Develop education resources and conduct training
• Report to the Chief Care Coordinator of YLDHC and assist in promotional activities or any other duty as assigned
• Night duty is not required, but may need to take on flexible hours and Saturday or Sunday duties on occasions
• May be required to rotate among various centres in Yuen Long

Requirements:

• Degree in Nursing and registered with the Nursing Council of Hong Kong
• At least 2 aggregate years of experience in patient care-related nursing practice as a Registered Nurse
• Working experience in Community or Medical Nursing will be an advantage
• Fluent in Chinese, English, and preferably, communicable in Mandarin
• Keen in teamwork and skilled in persuasive techniques
