The “Chronic Disease Co-care Pilot Scheme” was officially launched by the Primary Healthcare Office of the Medical and Health Bureau on 13rd Nov 2023.
Starting from the 14th to 16th of November, Pok Oi Hospital will host the ” Chronic Disease Co-care Pilot Scheme” Fun Day at the atrium of the ground floor of Chung Fu Shopping Centre in Tin Shui Wai. Pok Oi was honored to have Dr PANG Fei-chau, Commissioner for Primary Healthcare of the Medical and Health Bureau; Ms. Victoria Chan, Director of the Primary Healthcare Office of the Medical and Health Bureau; Ms. Christina Or, Senior Liaison Officer of the Yuen Long District Office; and Ms. Chan Wai-chen, Assistant Social Work Officer of the Yuen Long District Welfare Office of the Social Welfare Department, as the officiating guests of the kick off ceremony.

In his opening address, Dr. Chan Shou-ming, Chairman of the Board of Directors of Pok Oi Hospital, has emphasized the importance of the concept of “Life Course Prevention Care” and “Family Doctor Pairing” for the citizens in Yuen Long and Tin Shui Wai districts, hoping to connect with the community so that more citizens can receive the primary health services they need in the community.

Dr. Chan Kwok Kee, MH, JP, Chairman of the Medical Services Committee, the third Vice Chairman of the Pok Oi Hospital Board of Directors, expressed gratitude to the Bureau, centre colleagues, volunteers, family doctors and medical service providers for their invaluable assistances and contributions in the project.

The Pok Oi Hospital has been committed to – serving the community with the mission of ” We Love, We Care, We Serve “. Pok Oi hopes to leverage its robust regional network to enhance and fortify the provision of primary healthcare services. Yuen Long District Health Centre serves as a hub for primary medical services, offering people-centered services to citizens in the district. Citizens aged 45 years old or above who have no medical history of diabetes or hypertension are encouraged to join the government-funded ” Chronic Disease Co-care Pilot Scheme ” and pair up with family doctors for diabetes and hypertension screening. The list of family doctors participating in the program is posted on the ” Chronic Disease Co-care Pilot Scheme ” website and will be constantly updated.

The Yuen Long District Health Centre the ” Chronic Disease Co-care Pilot Scheme ” Fun Day held on November 14, 2023 .

Dr. PANG Fei-chau, Commissioner for Primary Healthcare of the Medical and Health Bureau, thanked Pok Oi Hospital for recognizing and supporting the “Chronic Disease Shared Management Plan”.

Dr. Chan Shou-ming, Chairman of the Board of Directors of Pok Oi Hospital, delivered his speech at the event that the hospital fully supports the implementation of the ” Chronic Disease Co-care Pilot Scheme ” at the Yuen Long District Health Centre.
